Hospital – Home Care Collaboration Project

IHA, the Home Care Association of New York State (HCA), and HANYS have developed a Statewide Hospital-Home Care Collaboration program as a means of facilitating innovative models of hospital-home care partnerships for front-end/pre-acute hospital care as well as far-end/post-hospital care, recovery, and long-term support.

This effort has focused on curating and hosting a series of webinars featuring prototypes of hospital and home care collaboration models that can be emulated by other providers statewide — working together, across settings.

This initiative is in its second year. To date, the series has delivered 11 webinars, with an additional 5 scheduled through March of 2022. Each webinar has featured a hospital and home care agency who have developed a unique partnership through the pandemic that has implications beyond a post -pandemic world.

A statewide summit: Models of Transformative Collaboration was held on December 2, 2021 and included panel discussions with NYS Department of Health, Vaccine hubs, the office of Mental Health, integrated primary care/mental health clinics and aging networks.
